First Appointment Paperwork:

If you have an initial consultation scheduled with Dr. Minter please download his intake paperwork, fill it out, and bring it with you to your appointment.

• Informed Consent for Treatment  This will be covered at the end of the 1st or 2nd appointment for those who wish to enter treatment with Dr. Minter after the initial evaluation or "getting to know you" conversation.

Please Be Aware . . . 

Over the last 3 years, Dr. Minter has had an increasing number of potential new clients who no-show scheduled first appointments.  This creates difficulties for existing clients who are waiting for those times.  As a result, our office now requires a credit card number to hold a first appointment slot.  This number will not be charged unless the appointment is cancelled within 24 hours or no-showed.   Timely cancellations, that is, with 24 business hours notice, do not incure charges.  Debbie will explain this policy more fully when you call.
