If you have an initial consultation scheduled with Dr. Minter please download his intake paperwork, fill it out, and bring it with you to your appointment.

Please Be Aware . . . 
Over the last year, Dr. Minter has had an increasing number of potential new clients who no-show scheduled first appointments.  This creates difficulties for existing clients who are waiting for those times.  As a result, Dr. Minter now requires a credit card number to hold a first appointment slot.  This number will not be charged unless the appointment is cancelled within 24 hours or no-showed.   Debbie will explain this policy more fully when you call.
COVID-19 Information
While video appointments are available, Dr. Minter sees most clients face-to-face. We want you to feel comfortable and safe in this environment.  His office is located in a small two-story building housing other professionals, including engineers, financial planners, and construction companies.  The landlord has elected not to require masks inside the building, but you are free to wear one if you have an underlying health concern or simply if you feel more comfortable.  Dr. Minter does not customarily wear a mask.  All hard surfaces in the office are disinfected regularly.  We observe social distancing within the waiting area and office.  If Dr. Minter tests positive for COVID, he will notify you and reschedule your appointment.  We ask that, in turn, you notify us and change your appointment if you are symptomatic or believe you have been exposed.
